COVID Office Policy

Dr. Fialk is fully vaccinated against SARS-CoV2 and is up to date with his boosters.

Appointment Cancellation Policy

As a courtesy, we ask that you please notify the office via phone, email, or patient portal message at least 2 business days prior to your upcoming appointment. Failure to do so may result in a $50 fee.

This fee may also be applied if you arrive 15 minutes late or more to your scheduled appointment as this may result in cancellation of the appointment. We ask that you please contact the office to notify us if you know you are running late.

After Hours & Correspondence Policy

Dr. Fialk prioritizes being accessible to all patient questions and concerns, however, he is not on-call after hours. Please do not hesitate to contact him on the patient portal, email, or leave a message on the office phone with questions/concerns regarding scheduling or other brief questions/concerns and he will get back within 24 hours during regular business hours.

 

More for involved questions/correspondence, Dr. Fialk's fee for medical decision making out side of an office visit (ie. review of medical records, labs or diagnostic imaging, spending time researching, updating a treatment plan), varies depending upon the time spent and is based on a $200 hourly rate.

Emergency & Urgent Care Services 

Emergency and urgent care services are not offered at Wellspring Integrative Health & Acupuncture. It is in your best interest to have a primary care physician with hospital admitting privileges who is familiar with your health history, if the need for hospitalization ever occurs. For this reason we encourage people to establish with an MD or DO primary care provider who has hospital admitting privileges and provides urgent care services, if they do not have one already.

Payment for services

Payment is due in full at time of service. Deductibles, co-pays, and co-insurance are always due at each appointment. For those who have insurance policies that do not cover visits or services offered, there is a discounted time of service fee schedule. 

 

We accept cash, checks, credit cards (Visa and Mastercard, Discover, American Express), Apple Pay, Venmo, as well as Health Savings Account cards (HSA) and Flexible Spending Accounts cards (FSA). Please make checks payable to Wellspring Integrative Health & Acupuncture.
